A factual guide to the medicines listed below, their regular charges and key safety considerations. A clinical assessment is needed to decide whether any medicine is suitable for you.
Who may be considered?
For adult weight management, the medicines listed here are licensed for an initial BMI of 30 or above, or 27 to below 30 with at least one weight-related health condition. Examples include high blood pressure, abnormal cholesterol, sleep apnoea, prediabetes or type 2 diabetes.
They are used alongside a reduced-calorie diet and increased physical activity. Meeting a BMI threshold does not automatically mean a medicine is suitable or that a prescription will be issued.
Already receiving treatment?
Your clinician will review your initial BMI and treatment history as well as your current health, progress and side effects. A lower current BMI does not, on its own, decide whether treatment should continue.
Considering the wider options
Weight management may involve support with eating patterns, activity and behaviour, medicines when appropriate, or referral for specialist care. Your circumstances and preferences guide the discussion.
BMI is one part of the assessment. Health risks can occur at lower BMI levels in some ethnic groups. NICE risk classifications and NHS access criteria are different from a medicine’s licence; they do not automatically change the licensed thresholds above.
This page focuses on adults. Some formulations have additional licensed uses or age groups; this does not establish eligibility for a Ways service.

30 tablets · one tablet daily · 30 days
A listed charge does not confirm stock or that a medicine will be suitable for you. The clinician decides the medicine, dose and any dose changes after assessment. For the 7.2 mg Wegovy injection dose, ask the pharmacy to confirm the quantity and device supplied for the four-week charge.
How the formulations differ
The following is a brief overview, not an individual dosing plan. Follow the prescription and the patient leaflet supplied with your medicine. Do not change doses or switch formulations yourself.
Mounjaro injection
Tirzepatide is injected under the skin once weekly. Dose increases are gradual and depend on the prescribed plan, response and tolerability.
Wegovy injection
Semaglutide is injected under the skin once weekly. Doses increase gradually. The 7.2 mg weekly dose is an option only when clinically appropriate for adults with obesity; it is not a starting dose.
Wegovy tablets
Semaglutide is taken as one tablet daily, swallowed whole after a recommended fast of at least 8 hours, with up to 120 mL water. Wait at least 30 minutes before food, drink or other oral medicines.
Foundayo tablets
Orforglipron is taken as one tablet daily, swallowed whole. It can be taken at any time of day, without food or water restrictions. Do not combine tablets to make a higher dose.
Important safety information
Before treatment, discuss your health history, allergies, pregnancy plans and every medicine or supplement you take. Read the current patient leaflet for the complete warnings and possible side effects.
Common side effects
Nausea, vomiting, diarrhoea, constipation and abdominal discomfort can occur. Symptoms may be more noticeable during dose increases. Contact your clinician if symptoms are severe or persistent.
Persistent severe abdominal pain
Stop the medicine and seek urgent medical help for severe abdominal pain that does not go away, particularly if it spreads to your back, with or without nausea or vomiting. This can be a sign of pancreatitis.
Serious allergic reactions
Call 999 for sudden swelling of the lips, mouth, tongue or throat, difficulty breathing or swallowing, or fainting and being unable to wake. These can be signs of anaphylaxis.
Vomiting and dehydration
Vomiting or diarrhoea can cause dehydration and kidney problems. Seek urgent advice from NHS 111 if you keep vomiting and cannot keep fluids down.
Other medicines and supplements
Do not combine or switch weight-management medicines yourself. Insulin or sulfonylureas may need adjustment to reduce low-blood-sugar risk. Foundayo has additional interactions; all medicines and herbal products need checking.
Surgery or deep sedation
Tell your surgical team and anaesthetist about treatment in advance. Delayed stomach emptying can increase aspiration risk. Follow their individual instructions about medicines, food and drink.
Sudden changes in vision
For semaglutide, including both Wegovy formulations, sudden loss of vision or rapidly worsening eyesight needs urgent medical attention.
Tell the clinician about previous pancreatitis, gallbladder disease, severe stomach or bowel problems, kidney or liver disease, and diabetes-related eye problems. These require individual assessment; this is not a complete list of exclusions.
Pregnancy, contraception & breastfeeding
These medicines should not be used during pregnancy. If pregnancy occurs or you think you may be pregnant, contact your prescriber promptly. Plan treatment changes before trying to conceive; the advice differs by medicine.
Mounjaro
Stop at least 1 month before a planned pregnancy.
If using oral contraception, switch to a non-oral method or add a barrier method for 4 weeks after starting and after each dose increase.
The current UK SmPC says use during breastfeeding may be considered. This requires an individual clinical discussion; it does not mean that it is appropriate for everyone.
Wegovy · both formulations
Stop at least 2 months before a planned pregnancy.
Use contraception during treatment. Wegovy does not have the same fixed post-dose-increase contraception instruction as Mounjaro or Foundayo.
Wegovy should not be used during breastfeeding.
Foundayo
Stop at least 3 weeks before a planned pregnancy.
If using oral contraception, switch to a non-oral method or add a barrier method for 30 days after starting and after each dose increase.
Foundayo should not be used during breastfeeding.
Vomiting or severe diarrhoea can affect oral contraception. Follow your contraceptive leaflet and ask a healthcare professional for advice. Do not assume that one medicine’s contraception advice applies to another.
